Grant Fiscal Forms
The OTS Claim forms are provided to grantees during the pre-operational meeting. Contact your Regional Coordinator for claim forms customized for your grant or follow the Claim Spreadsheet Instructions (doc) before downloading grant claim forms.
The OTS Claim Forms are provided in a zipped (compressed file) format. After you download the appropriate claim form, it will need to be extracted.
To do this, you can use a file compression program like WinZip©. If you do not have a file compression program, you can go to the download site for Winzip to download and install a free evaluation version of WinZip.
